The SGTPL-2516-0001P from Vishay is a Space-Grade Planar Transformer that is designed for high reliability space grade switch mode power supplies, flyback converters and forward converters applications. This transformer requires a primary input voltage of 22 - 36 V and  provides a secondary output voltage of 5 - 12 V. It has an inductance of 30 μH and a turns ratio of 7: 2: 4.5.
